Functional medicine utilizes an in-depth history and a physical exam, combined with standard and advanced diagnostic testing, in order to better understand the root causes of your medical conditions. The aim of functional medicine is to add to your life those factors which promote health and remove from your life those factors which promote disease. Our goal is to intervene as far upstream as possible (addressing the root causes) instead of focusing on treating the downstream consequences of disease.
All too often we ignore our health until disease develops and then do little to address the primary factors which have led to our failing health. This does not have to be your story. Could you benefit from Low Dose Immunotherapy?
Excessive inflammation is at the root of many chronic diseases. Low Dose Immunotherapy (LDI) can benefit those with an overactive immune system by helping to reestablish tolerance to different allergens, chemicals, and organisms. Our immune system may inappropriately attack substances that should not trigger an immune response, leading to food allergies and sensitivities, environmental allergies, chemical sensitivities, and autoimmune diseases. Furthermore, an overly reactive immune response to microorganisms in our body can contribute to chronic illness. This can happen with organisms that cause persistent symptoms, such as those related to chronic Lyme disease, Lyme co-infections, herpes, yeast, and acne as well conditions caused by other bacteria, yeast, mold, and viruses. LDI works by administering a very low dose of an antigen under your tongue to which your body is overly reactive. Initially it is administered about once a week with each subsequent dose increasing in concentration until you reach a dose which elicits a response determined by a noticeable change in your usual symptoms, usually within 24 - 48 hours of your LDI dose. Once the correct dose is achieved it may need to be repeated as frequently as every 4 – 8 weeks, with the intervals often increasing as immune system tolerance continues to improve. Could you benefit from neuromodulation?
Newer technologies are now available to help address the root causes of many neurologic, psychiatric, and/or learning disorders which arise from biochemical and electrical abnormalities in the brain. We offer neurofeedback (brainwave biofeedback) and a variety of neurostimulation therapies, including transcranial photobiomodulation (tPBM) using near-infrared LEDs, transcranial direct current stimulation (tDCS), transcranial alternating current stimulation (tACS), and pulsed electromagnetic field (pEMF) stimulation. To determine if one or more of these therapies could be helpful to you, we first perform a 3D quantitative EEG map of your brain's function in our office. If any abnormalities are found that correlate with your symptoms, a personalized protocol is developed with the goal of improving brain function in those specific dysregulated regions and networks of the brain. We provide in-depth consultations focusing on the evaluation, treatment, and prevention of chronic disease in adults and children."Cornerstone Functional Medicine is not a replacement for your primary care physician. Dr. Lewis works as a functional medicine consultant in conjunction with your current health-care team to assist you in understanding why you have become ill and to expand your diagnostic and treatment options to optimize your health. In order to be one of our patients, you are required to have your own primary care provider who will address your acute health care problems, immunizations, routine health care screenings, and annual physical exams. Your current health care providers will continue to prescribe your current medications which they have initiated for as long as you have need of them.

Our consultations are not for treating emergent or acute medical conditions.If you have a medical emergency, an acute illness, recent trauma, chest pain, or other acute medical conditions, then conventional medical care is the right solution for you. With many years of experience as a family physician and emergency department physician Dr. Lewis can attest to the great value these services render for acute medical problems. Our practice has the goal of helping you to minimize your need for acute care and emergency services. In the event of an urgent or emergent condition either contact your primary care provider, be seen in an urgent-care facility, go to an emergency department or call 911.

We provide telemedicine consults.Telemedicine Policy when there is low risk for COVID-19 transmission During times when the risk of contagious disease is at a low level we will schedule your first consultation on-site in the office with the option of follow up visits being provided by either video calls or over the phone. Because Important information can be obtained through doing a physical examination, Dr. Lewis prefers to see you in our office for the first consultation visit. Under extenuating circumstances (poor traveling conditions, sickness, etc.), you could have your first consultation visit over the phone or by videoconference, with your second consultation visit taking place in our office. After your in-office consult, many of the follow-up visits can be handled through telemedicine. For those who desire visits for more than one year we advise that you be examined in our office once a year. Telemedicine Policy under COVID-19 High Risk Conditions Dr. Lewis normally prefers that the first office consultation take place in his office; however, in order to reduce the risk of transmitting the COVID-19 infection, if the risk of the spread of COVID-19 infections returns to a high risk state, then all visits can be handled through telemedicine by video calls or phone calls unless the benefits of an in person exam outweigh the risks.

Our office feesOur fee schedule is as follows: The preliminary 15-minute phone call: Free The first consultation visit (The initial intake): $500 The consult fee includes the 60-75 minutes spent directly with Dr. Lewis as well as his time before and after your visit reviewing your functional medicine analysis, initial intake forms, and any submitted medical records, and developing your initial diagnostic evaluation and preliminary treatment plan. The second consultation visit (Lab review and treatment plan): $500 This fee covers the 60-75 minutes spent directly with Dr. Lewis as well as his time prior to your appointment analyzing the results of your lab studies and fine tuning your personalized treatment plan. The third consultation visit (Follow up): free 30-minute visit There is no cost for the third visit. Subsequent follow-up visits: $ 150 for every 30 minutes. These fees are paid at the time of your visit. These follow-up visits are charged at a rate of $50 for every 10 minutes. These appointments usually last between 30-60 minutes depending on the complexity of your situation and your particular needs. Low Dose Immunotherapy The cost is $175 for 5 doses of the LDI. This includes Dr. Lewis’ reviewing your portal LDI Dose Reports and providing portal message feedback regarding directions for each subsequent dose. It also includes the preparation and mailing of your customized LDI doses. If Dr. Lewis tells you not to take some of the doses which were mailed to you due to a flare in your symptoms or due to the need to fine tune your doses, your new doses replacing the unused doses will be provided at no additional cost. If an office visit, telemedicine visit, or phone call with Dr. Lewis is needed, you will be charged at our usual rate for these services as described above. 3D Brain-function mapping using quantitative EEG: $100. This covers the cost of obtaining and interpreting the scan. It is not covered by insurance. Neurofeedback, neurostimulation, and/or Frequency Specific Microcurrent treatments administered in the office: $100 for sessions that last 30-60 minutes. In the exceptional situation where the treatments last more than 60 minutes the fee is $150 for 61 - 90 minutes and $200 for 91 - 120 minutes. It is unusual for insurance to cover these procedures. How do I get to your office?When coming from Titusville : Head south on Route 8 for about 3 miles. After you pass Fairview Road you will see a cell phone tower on your right. Turn right onto Sopher Road about 100 feet beyond the tower. Drive for 0.8 miles on Sopher Road during which time you will pass one driveway on your right. Turn at the second driveway on the right at 233 Sopher Road, where you will see the signs identifying our practice. The office is located at the end of the driveway on the right-hand side where you will see another sign identifying our office. When coming from Oil City: Head north on Route 8 for 11 miles. After you pass Buxton Road you will see a cell phone tower on your left. Turn left on Sopher Road about 100 feet before you reach the cell phone tower. Drive for 0.8 miles on Sopher Road during which time you will pass one driveway on your right. What are Dr. Lewis' qualifications to practice functional medicine?Dr. Lewis is an Institute for Functional Medicine Certified Practitioner and is board-certified in neurofeedback. In addition, he has completed a residency and a fellowship in family medicine, has been board-certified in family medicine since 1988, and has had over 30-years experience in family and emergency medicine.

Will you bill my insurance company?We do not have contracts with any insurance companies, and for this reason we will not be billing your insurance company. You will be given an invoice which you can submit to your insurance company (at the address typically found on your insurance card) after your visit with us if you desire. Our practice will be treated as an out-of-network provider. Your reimbursement will be dependent on your individual insurance plan. The cost of the office visits can be applied to your deductible and can be paid for with your Health Savings Account or Flexible Savings Account if you have one.

Which of my medical records should I send you prior to my first consultation visit?At least seven days prior to your first consultation visit we request that you provide us with pertinent medical records in order to help to better understand your medical condition. Please send them to us either through the online patient portal (to which you will have access) or you can fax, mail, or hand deliver them to us at the office so they can be reviewed prior to your appointment. (Click on the following links to read the Privacy Policies for our office and our secure online web portal, Cerbo's MD-HQ.) The following information would be very helpful but is not required: A recent medical summary of your medical records from your latest medical providers and specialists. Labs and medical test summaries performed within the last year. Only include studies performed over one year ago if you believe they are especially important. Summary of treatment regimens, therapies, medications, and supplements taken or utilized over the last year and any earlier ones which you believe to be pertinent. If you have been admitted to the hospital, the most recent admission note and discharge summary would be helpful. Genetic testing results. Do not send: Notes from routine medical office visits. Diagnostic imaging films or discs. (We will take the radiologist's interpretation of the studies if you believe the studies are pertinent.) Labs and test results that are over one year old, unless you feel that they are important to understanding your medical conditions. Please do not send more than 50 pages of medical records. The records will be scanned, stored securely in our electronic medical records, and then shredded.

What do I need to do prior to my first consultation visit?After your free 15-minute phone call, if you decide that you are interested in becoming one of our patients you will be able to schedule your first consultation visit. You will be sent an email with a link to create an account in your Cerbo MD-HQ patient portal. Please follow the instructions you will receive regarding what information we need prior to your first appointment and inform us when you are done. To complete the questionnaires for the Cerbo MD-HQ web portal please click on the "Questionnaires" icon on the right side of your portal webpage. Dr. Lewis will need this information to identify the various factors that have put you at high risk for illness, those triggering events throughout your life which have affected your health, and various ongoing factors that are currently contributing to your medical conditions (i.e., the root causes), all of which have led to your current health concerns. Please allow enough time to accurately answer these questions because the information will be critical for Dr. Lewis to analyze prior to your first visit. At least one week prior to your scheduled first consultation visit, please also submit your pertinent medical records according to the guidelines noted in the question and answer above. Our fax number is (626) 317-8116. If you are mailing medical records please send them to Cornerstone Functional Medicine, 233 Sopher Road, Titusville, PA 16354. On the day of your visit, bring all of your medications and/or your supplements in their original bottles. We would encourage you to bring a notebook and take notes during our visits. What is your policy for canceling, changing or missing appointments?We do not double-book patient appointment times as is common in other offices and have reserved your time slot for you. If you want to cancel or reschedule your appointment, please be courteous and notify us within 48 business hours of your scheduled appointment. This helps ensure that we can meet the needs of all of our patients who are waiting to get an appointment. If you cannot travel to the office due to weather or other factors we can change the visit to a phone or video conference call at your previously scheduled time.

What are your email policies?Due to the increasing number of electronic communications, and in order for you to receive a response in a timely manner, please limit your messages to: Questions about a medication, supplement, or diagnostic test recommended by our practice; Requests to refill a medication or supplement; Possible side effects of a treatment; Responses to a question from our staff; or, Notes concerning scheduling, canceling, or changing an appointment. Your question should be brief, concise, and, ideally, answerable with a “yes” or “no.” If your questions or concerns are beyond the scope described above, and/or require more time and attention, you will be asked to schedule a follow-up appointment with Dr. Lewis. Our email address admin@csfmedicine.com is a secure email platform; however, if your email address is not secure (which is the norm) we cannot guarantee the privacy of what you communicate to us by email. That is why we request you contact us through our secure Cerbo patient portal with private health information when you become one of our patients. Do you prescribe medications?Our main focus is on the improvement of your health. If Dr. Lewis believes that one or more prescription medications would benefit you, he will discuss that with you and write for the prescriptions as needed. He requests that you inform all of your health care providers of all changes to your medications and supplements during your next visits with them. Either Dr. Lewis or your primary care provider can refill medications which he has initiated, but all of your other medications will need to be refilled by your primary care provider and/or the specialists who initiated those medications. He will not be prescribing narcotics or other controlled substances. However, he will work with you to try to determine what has been causing your need for these medications and look for ways to decrease or eliminate your need for them.

Do you recommend nutritional supplements?Dr. Lewis prefers using diet, lifestyle changes, and nutritional supplements, instead of pharmaceutical medications, when they can more effectively address the root causes of your medical conditions.

What do I do if my symptoms are getting worse or I believe I may be having side effects from my prescribed treatments?If you believe you are experiencing a medical emergency, call 911 or go to the emergency department. For non-emergent situations contact our office by phone or by email through our secure with portal. If you are having an adverse reaction to a supplement, stop it and notify our office (or call 911 if it is an emergency). Some prescription medications need careful monitoring when they are discontinued. For this reason, if you believe you are having a mild adverse reaction to a medication prescribed by Dr. Lewis, you should contact the office prior to discontinuing it. However, if you are having a moderate reaction to a medication, you should stop the medication and contact our office. If you are experiencing an emergency or a severe reaction to a medication or supplement, immediately call 911 and stop using the medication.

My labs are back. Can you send me the results without the need for a follow up visit?The implications of having both normal and abnormal labs are often complex and dependent on multiple factors. We require patients to schedule follow-up visits with Dr. Lewis for the review of lab work so that all diagnostic and therapeutic options can be discussed and a customized treatment plan can be initiated or adjusted.

My labs are back. Can you discuss them with me over the phone or by email?Lab result discussions are reserved for scheduled appointments (phone, video, or in-office) unless Dr. Lewis has determined that the lab results are of an urgent or emergent nature and need to be addressed prior to your scheduled follow up visit. In the event that your lab work needs urgent attention you will be contacted by phone or email depending on the urgency.

I am going to run out of my supplements or medications prior to my appointment. Can you tell me if I am going to be continuing on this supplement or medication?Unless Dr. Lewis instructed you otherwise, we recommend that you continue your supplements or medications until your next scheduled appointment at which time you can discuss your ongoing need for them with Dr. Lewis. You can obtain refills of your supplements on your own. Your pharmacy can fax refill requests to our office at (626) 317-8116.

Why don't you bill insurance companies?Currently, insurance companies do not reimburse for the amount of time that functional medicine doctors put into your comprehensive medical evaluation and customized treatment plans. For this reason the practice could not be sustained if we attempted to cover our costs based on current insurance reimbursement rates. Even tradition medical practices which typically provide relatively brief office visits are increasingly not financially self-sustaining due to decreasing insurance reimbursement rates and have many have needed to merge with hospitals or large medical groups in order to cover their costs.

Will my insurance cover the lab work ordered by Dr. Lewis?Dr. Lewis orders lab work from various laboratories. Insurance companies can vary widely on which tests and which laboratories they will cover or partially cover. Please call the phone number for your insurance company (which is on the back of your insurance card) to verify coverage for lab work and blood drawing fees.

Will my insurance cover the supplements or prescriptions ordered by Dr. Lewis?As a general rule insurance companies will not cover the cost of supplements. For prescription medications, please call the phone number for your insurance company (which is on the back of your insurance card) to verify coverage. If you have a health savings account or flexible spending account you can usually use these funds to pay for prescriptions, supplements, and lab work ordered by Dr. Lewis.

My insurance company will not cover the prescription medication ordered by Dr. Lewis. What should I do?You will need to contact your insurance company and find out from them what alternative medications, if any, are covered. If they will cover an alternate medicine, then contact our office with that information.

Do the fees charged by Cornerstone Functional Medicine cover the costs of labs, supplements, or prescriptions? "Our fees only cover the cost of your consultation visits and the time Dr. Lewis spends reviewing and customizing your care prior to and following your office visits. They do not cover the costs of labs, supplements or prescription medications. The specialty functional medicine testing and supplements are completely optional. We will work with you to come up with a customized diagnostic and treatment plan that works best for you.

Will you complete my disability paperwork?You will need to see your primary care provider for this.

Do you perform workers compensation evaluations?We do not. However, we would be glad to provide you with a functional medicine approach to any of your chronic health conditions whether they are work related or not.

What is the patient portal?All of our new patients will be emailed a link to our patient portal. The patient portal is a secure website which enables us to communicate with you confidentially. It is the website which you will use to complete a functional medicine assessment which will provide Dr. Lewis with invaluable information in determining the root causes of your conditions and assist him in developing a diagnostic and treatment protocol.
